Remember,  you have to turn the features on at the Site Collection level AND at the site level....   Had the same problem that you did, but as soon as I turned them on at the site level, everything was created  (except the login aspx page which was downloaded from this site separately).&lt;br /&gt;

Had the same issue myself.  I just copied the document to my desktop and opened it with MS Word.  Worked like a charm.  Must be some &amp;quot;gotcha&amp;quot; in the file that stops viewer from working...&lt;br /&gt;

Hi, great idea ...... i hope i can get it work.&lt;br /&gt; &lt;br /&gt;I&amp;#180;ve done everything step by step as you write it in the instructions and double checked everything, but i get a strange message if i try to login with any user i created in the Site Member List:&lt;br /&gt; &lt;br /&gt;   &lt;br /&gt;&lt;b&gt;The ValidateUser function has thrown the following error One or more field types are not installed properly. I'm using the Word Viewer and Office Compatibility pack.  Does anyone else have trouble opening the deployment guide?&lt;br /&gt;

NM - I got it going, I needed to fix my webconfig, I did not have the correct list specified as the &amp;quot;source&amp;quot; for the member list. beyond that, I just copied the login page provided under the layouts folder  and pathed to it in the web.config and it is good to go. be sure to mark your users as active or they wont work. i did not have any luck with the master user settings in web.config either - i just left everything as windows auth went in and setup a couple users in the list then flipped it back to forms auth.  still cant seem to make sense of the roles piece. &lt;br /&gt;

Sorry for not getting this, but how do I deploy the membershiplogin.aspx into the sharepoint site? &lt;br /&gt;
