when submitting a new Code Review Response defualt the Area and Iteration values of that new work item to the values of the Associated Item's Area and Iteration

In the New Code Review Response form and the Replay Code Review form prepend the Work Item ID to the Title for easier finding of work items

Replay is off because it is using TextSelection.TopLine instead of TextSelection.TopPoint.Line in ReplayLink.HighlightCodeSnippet()

If Visual Studio goes online or offline it seems like the status of the active TFS project should change and that TeamReview should respond appropriately.
